Job summary

Trinity Medical Centre (Stoke-On-Trent) is seeking an Advanced Nurse Practitioner to work autonomously in a primary care setting. You will assess, diagnose, treat and manage patients across ages, using advanced clinical skills in line with NMC standards and local governance.

The role includes comprehensive assessments, safe prescribing, referrals, and contributing to service improvement within the practice.

Qualifications

• Registered nurse with NMC registration and advanced practice qualifications.
• Masters-level education in advanced practice or equivalent.
• Independent/Supplementary Nurse Prescriber (V300) credentials held.

Responsibilities

• Undertake comprehensive clinical assessments and consultations.
• Assess, diagnose, and manage patients with acute and chronic conditions.
• Prescribe safely within scope of practice and guidelines.
• Refer patients to secondary care when required.
• Maintain accurate clinical records and contribute to governance.
