Job summary

Hodnet Medical Centre in rural North Shropshire is seeking an Advanced Nurse Practitioner to join our friendly team. You will provide autonomous clinical assessment, diagnosis, treatment and ongoing management of patients with a broad range of conditions, working closely with GPs, pharmacists and the wider practice team.

Qualifications

• Registered with NMC/HCPC and qualified in advanced practice.
• Certified Independent prescriber.
• Advanced clinical assessment skills.

Responsibilities

• Provide autonomous clinical assessment, diagnosis, treatment and ongoing management of patients with a broad range of acute and chronic conditions.
• Collaborate closely with GPs and wider multidisciplinary team to deliver safe, holistic patient care.
• Manage patient blood results and follow-up appointments with training and support.
• Mentor to develop a wider caseload and prescriber capabilities within the practice.
