Job summary

Swadlincote Surgery is seeking an Advanced Nurse Practitioner to work one day per week on a permanent basis. The role focuses on autonomous face-to-face consultations, diagnosing, managing, prescribing and discharging patients of all ages within a multidisciplinary team and active PCN.

Independent prescriber status and an advanced practice qualification are required. The post offers opportunities to develop a specialist interest, contribute to QOF targets, and maintain high standards of patient

Qualifications

• Advanced Nurse Practitioner qualification at master's level, or a postgraduate diploma or degree in advanced practice
• Minor illness qualification
• Qualified independent nurse prescriber
• Meets the standards for registered ANP working at advanced level
• Evidence of autonomous physical examination and assessment skills in practice
• Good understanding and experience of chronic disease management, primary prevention and screening services
• Broad and sound knowledge of complex and common primary care conditions in adults and children
• Excellent interpersonal and communication skills, handling information in a confidential manner
• Commitment to lifelong learning and audit to ensure evidence-based practice

Responsibilities

• Diagnose, manage, prescribe for and discharge patients of all ages with illness or injury through face-to-face consultations
• Manage acute and chronic medical conditions, advising as subject matter expert and prioritising health issues
• Identify, manage and support patients at risk of developing long-term conditions
• Provide advanced specialist nursing care in line with evidence, NICE and NSF guidance
• Request, process and interpret pathology and other test results
• Develop and embed health promotion and wellbeing programmes, including opportunistic promotion
• Act as an extended and supplementary prescriber and support patients with prescribed and over-the-counter medicines, reviewing annually as required
• Recognise, assess and refer patients with mental health needs or substance misuse issues
• Liaise with external services to support vulnerable patients
• Support the clinical team with safeguarding and clinical emergencies
• Maintain accurate clinical records and use SNOMED CT codes effectively
• Contribute to QOF and other practice targets, and delegate clinical tasks within scope of practice
• Nice to have: Experience of working in primary care
