Integrated Care 24 (IC24) is seeking an Advanced Clinical Practitioner for Home Visiting. You will assess, diagnose and treat patients in their own homes, using advanced clinical skills to provide urgent care and appropriate care plans, while maintaining high standards of safety and patient-centred care.

Qualifications

• Hold current professional registration with NMC, HCPC or GPhC.
• Hold a Masters Degree / Level 7 in Advanced Clinical Practice or be working towards one.
• Hold an Enhanced Assessment and Consultation qualification at Level 7.
• Hold a Minor Illness qualification at Level 7.
• Be a registered Independent Prescriber.

Advanced Clinical Practitioner - Home Visiting, Permanent | Part time hours available

Bring advanced urgent care to patients who need to be seen at home. Not every patient who needs urgent care can come to us. Some are housebound, frail, approaching the end of life or have been clinically assessed as needing a face‑to‑face review in their own home. What they need is an experienced clinician who can assess the whole picture, make confident autonomous decisions and put the right care in place. At IC24, we’re looking for an Advanced Clinical Practitioner to join our Home Visiting Service. You’ll work autonomously in patients’ homes, using advanced assessment, examination, diagnostic and prescribing skills to provide safe, effective and patient‑centred urgent care. You’ll care for patients across the full General Practice population, including children, older people, frail patients and those with mental health or end‑of‑life needs. If you enjoy clinical autonomy, variety and the challenge of making safe decisions outside a traditional clinical environment, this could be the role for you.

What you’ll focus on

Assessing and treating patients in their own homes. You’ll provide face‑to‑face assessment, diagnosis and treatment for patients who have been clinically triaged as needing a home visit. You’ll take a detailed history, carry out a physical examination and use advanced clinical reasoning to establish a differential diagnosis and agree the most appropriate treatment or care plan. You’ll see a wide range of urgent and primary care presentations, often without immediate access to the investigations and resources available in a hospital or clinic.

Making confident autonomous clinical decisions

Home visiting requires strong clinical judgement. You’ll often be assessing patients you’ve never met before, sometimes with limited information and sometimes in complex or emotionally difficult circumstances. You’ll need to be comfortable managing uncertainty, prioritising clinical risk and deciding whether someone can be safely treated and supported at home or needs escalation to another service. You’ll always work within your own competence and know when to seek further input from a GP or Clinical Consultant.

Treating the whole patient

Seeing someone in their own home gives you a different perspective on their health. You’ll care for patients across the full spectrum of General Practice, including newborns and children, adults, older and frail patients, people with mental health needs and those approaching the end of their lives. You’ll consider the patient’s physical health alongside their environment, support network and wider social circumstances. That means looking beyond the immediate symptoms and understanding what the patient genuinely needs to remain safe.

Prescribing and managing treatment at home

As an independent prescriber, you’ll develop and deliver evidence‑based treatment plans. You’ll prescribe safely within agreed clinical guidance, review existing medication where appropriate and make sure patients and carers understand what happens next. Where required, you’ll undertake point‑of‑care testing, arrange appropriate investigations and interpret available clinical information to support your decision‑making.

Keeping patients safely in their community

One of the most important parts of home visiting is helping patients receive the right care in the right place. Where clinically appropriate, you’ll help patients remain safely at home by putting appropriate treatment, referrals, advice and safety‑netting in place. You’ll work with primary care, community nursing, pharmacy, social care, ambulance services and secondary care where needed to support continuity of care. And when hospital or another service is the right place for a patient, you’ll recognise that and upscale appropriately.

Supporting palliative and end‑of‑life patients

Some of the most important visits you’ll make will be to patients approaching the end of their lives. You’ll assess symptoms, prescribe and manage treatment where appropriate and work sensitively with patients, relatives and carers during what can be a difficult and emotional time. You’ll use your advanced clinical judgement alongside existing care plans and local guidance to help make sure patients receive compassionate, appropriate care.

Working independently, but never in isolation

Home visiting is autonomous by nature. You’ll spend a significant part of your working day out in the community, so you’ll need to be confident managing your own workload and making decisions independently. But you’ll remain part of a wider IC24 clinical and operational team. You’ll communicate with colleagues, carry out clear clinical handovers and work with other professionals to make sure patients receive joined‑up care.

Maintaining excellent clinical standards

You’ll work within IC24’s clinical governance framework and in accordance with the requirements of your professional registration. You’ll maintain accurate clinical records, contribute to audit and quality improvement and keep your clinical knowledge and skills up to date through CPD, supervision and reflective practice. You’ll also support colleagues and contribute to clinical supervision, coaching and development where appropriate. This is advanced practice beyond the consulting room. No two home visits will necessarily look the same. You’ll see patients with everything from relatively straightforward acute illness to complex physical, mental health and social needs. Sometimes the answer will be clear. Sometimes you’ll need to piece together limited information, understand the patient’s home circumstances and make an important clinical decision without the immediate support of a full clinical team around you. You’ll probably thrive here if you:

What you’ll bring

• Hold current professional registration with the NMC, HCPC or GPhC, with at least five years’ post‑qualification and registration experience
• Hold a Masters Degree / Level 7 qualification Advanced Clinical Practice or will be working towards one
• Hold an Enhanced Assessment and Consultation qualification at Level 7
• Hold a Minor Illness qualification at 7
• Be a registered Independent Prescriber
• Have advanced assessment, examination, diagnosis and treatment skills
• Have significant post‑registration experience including autonomous practice at an advanced level
• Be confident working independently in a patient’s home
• Have strong clinical reasoning and risk‑assessment skills
• Be able to work flexibly, including unsociable hours, weekends and Bank Holidays
• Experience of home visiting, palliative care, urgent care, primary care, verification of expected death, chronic disease management, clinical supervision, teaching, audit or service improvement would also be valuable

Part time hours available up to 20 hours per week
