Job summary

Integrated Care 24 (IC24) is seeking an Advanced Clinical Practitioner for its Home Visiting service. The role is permanent, part-time, and involves independent clinical decision-making in patients' homes to deliver urgent care.

You will assess, diagnose and manage a wide range of urgent and primary care presentations, including palliative and end-of-life care, with prescribing responsibilities and clinical governance under IC24 guidelines.

Qualifications

• Hold current professional registration with NMC, HCPC or GPhC.
• Masters Degree / Level 7 Advanced Clinical Practice.
• Enhanced Assessment and Consultation qualification (Level 7).
• Minor Illness qualification (Level 7).
• Be a registered Independent Prescriber.

Responsibilities

• Provide face‑to‑face assessment, diagnosis and treatment in patients' homes.
• Prescribe safely within clinical guidance and review medications as needed.
• Maintain clinical records, participate in audits and CPD.
