Job summary

Integrated Care 24 (IC24) is seeking an Advanced Clinical Practitioner for Home Visiting. You will assess, diagnose and treat patients in their own homes, using advanced clinical skills to provide urgent care and appropriate care plans, while maintaining high standards of safety and patient-centred care.

You will work autonomously within the IC24 clinical team, with opportunities to contribute to clinical governance, CPD and service development, including palliative and end‑of‑life care as

Qualifications

• Hold current professional registration with NMC, HCPC or GPhC.
• Hold a Masters Degree / Level 7 in Advanced Clinical Practice or be working towards one.
• Hold an Enhanced Assessment and Consultation qualification at Level 7.
• Hold a Minor Illness qualification at Level 7.
• Be a registered Independent Prescriber.
• Have advanced assessment, examination and treatment skills.
• Have significant post‑registration autonomous practice at an advanced level.
• Be confident working independently in a patient’s home.
• Have strong clinical reasoning and risk‑assessment skills.
• Be flexible to unsociable hours, weekends and Bank Holidays.
• Experience of home visiting, urgent care, primary care and palliative care would be valuable.

Responsibilities

• Assess and treat patients in their own homes with face‑to‑face assessment and treatment.
• Prescribe and manage treatment at home within clinical guidelines.
• Maintain clinical governance, records and CPD; collaborate with colleagues for continuity of care.
