Job summary

IC24 is seeking a Bank Advanced Clinical Practitioner to join its Primary Care team at Herstmonceux, East Sussex. The role offers a mix of face-to-face and remote consultations, with clinical administration and opportunities to contribute to service development within a supportive MDT.

You will work as an Autonomous prescriber, delivering safe, evidence-based care across a range of presentations and enabling timely patient access within the integrated care model.

Qualifications

• Registered healthcare professional with current NMC/HCPC registration.
• Holds an Advanced Clinical Practice qualification or equivalent.
• Qualified Non-Medical Prescriber.
• Has completed Physical Assessment of Adults training.
• Excellent clinical assessment, diagnostic and decision-making skills.
• Able to work autonomously within their scope of practice.
• Committed to clinical governance and CPD.

Responsibilities

• Deliver advanced clinical care, assess, diagnose and manage a wide range of conditions.
• Prescribe medications independently following clinical assessment and examination.
• Assess and manage complex patient presentations and determine appropriate pathways.
• Support long-term condition management and women's health.
• Contribute to quality and clinical governance through audits and learning.
• Collaborate with GPs, nurses and allied health professionals for coordinated care.
• Support service development and improvement initiatives.
