Job summary

Frimley Health NHS Foundation Trust is seeking an Associate Practitioner to join the Interim Care and Support Team (ICAST) in Fleet. You will support patients in their own homes while awaiting care packages and contribute to developing the team, with supervision and ongoing professional development.

The role requires a minimum Level 5 qualification, driving licence, and access to a vehicle for work. You will work a seven-day roster and participate in regular 1:1 supervisions and annual

Qualifications

• Foundation degree, NVQ Level 5 or evidence of working towards one of these qualifications, relevant to this role
• Previous experience as an HCA, Community Support Worker or Therapy Assistant
• Experience undertaking patient assessments and community work

Responsibilities

• Assess patients’ care needs and write care and support plans
• Empower patients to be as independent as possible using their existing support networks
• Deliver hands-on care to patients with high quality outcomes
• Work as part of a seven-day rota 08:00-20:00 with vehicle use for work purposes
