Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Rehabilitation Assistant for our Community Therapy Team covering Petersfield and Bordon. You will work with Occupational Therapists, Physiotherapists and other professionals to support patients at home and in community settings, delivering delegated rehabilitation programmes.
You will monitor progress, assist with mobility and daily activities, and ensure patients and carers are engaged in goal setting.
Are you passionate about helping people regain their independence and achieve their rehabilitation goals? We are looking for an enthusiastic and motivated Rehabilitation Assistant to join our friendly and supportive Community Therapy Team covering Petersfield and Bordon. Working alongside Occupational Therapists, Physiotherapists and other healthcare professionals, you will play a key role in supporting patients with a wide range of rehabilitation needs within their own homes and community settings. You will deliver delegated rehabilitation programmes, support mobility and functional activities, monitor patient progress and contribute to achieving positive outcomes for patients.
You will need excellent communication and organisational skills together with a compassionate, person-centred approach to care. Previous experience working within a health, social care or rehabilitation setting is desirable, although full support, supervision and development opportunities will be provided.
