Job summary

Frimley Health NHS Foundation Trust is seeking a Band 6 Clinical Practitioner to support the All-Age Complex and Continuing Care (AACCC) service. You will apply clinical judgement to assess complex needs, working with families and a broad care team to ensure appropriate, timely care.

The role focuses on NHS CHC assessments, professional decision-making, and expanding experience across the CHC pathway, with flexible hours and potential remote working options.

Qualifications

• Educated to degree level in relevant subject or equivalent level of experience with up-to-date registration with the relevant regulatory body e.g. NMC.

Responsibilities

• Undertake NHS Continuing Healthcare (CHC) assessments and reviews with multidisciplinary teams.
• Make evidence-based recommendations in line with the National Framework.
• Develop experience across CHC pathway including Fast Track, care planning and personal health budgets.
• Work flexible hours with opportunities to work remotely while maintaining clinical autonomy.
