Job summary

West Sussex County Council seeks an Assistant Care Manager to support discharge planning and care coordination across Acute ward, A&E and Community Hospitals. You will assess individuals, develop and monitor care plans, and work with teams to ensure safe, timely discharges and person‑centred support at home.

You will document outcomes, communicate with diverse audiences and maintain records. The role requires initiative, resilience and a flexible approach to out-of-hours work and county travel.

Qualifications

• Ability to analyse and interpret information to prioritise needs and make recommendations.
• Ability to work independently and as part of a team under pressure and tight deadlines.
• Experience working in a multi-agency environment to deliver services.
• Ability to communicate complex information to diverse audiences clearly.
• Sound IT and keyboard skills.
• Recognise limits of own accountability and seek support and supervision.

Responsibilities
