West Sussex County Council is seeking an Assistant Care Manager to join the Sensory Services team. The role involves planning and delivering person-centred support for adults with sensory impairments, including home visits and risk assessments.

You’ll work in a countywide setting with partners to empower individuals and promote independence. The position is permanent, 21.5 hours per week, with office base Horsham and hybrid working (2 days in the office per week) after interview.

Qualificações

• Understanding the impact of disability, especially sensory impairment, on wellbeing.
• Ability to complete a strengths-based assessment as per Care Act 2014.
• Ability to develop a person-centred care and support plan with defined outcomes.
• Excellent communication and interpersonal skills; able to use information technologies.
• Ability to prioritise, organise work and meet deadlines.

Responsabilidades

• Develop and implement tailored support plans to improve wellbeing and deliver positive outcomes.
• Provide advice, information, and assistive equipment or independent living skills.
• Visit people in their own homes and conduct high-quality risk assessments, including MCA reviews.
• Respond to safeguarding enquiries and work with a dedicated team to promote safety.
• Ensure compliance with policies and legislation applicable to the role.
• Participate in an out-of-hours duty system.
• Travel across the County to visit clients in their homes, including locations not easily reached by public transport.
