Job summary

West Sussex County Council is seeking an Assistant Care Manager to join our Sensory Services team. You will develop personalised care plans, assess risks, and support adults with sensory impairments to live independently while coordinating with partners.

Travel across the county is required to visit service users at home, undertake safeguarding and MCA assessments, and ensure compliance with policy and legislation. A compassionate, organised approach is essential.

Qualifications

• GCSEs including English and Maths or equivalent.
• NVQ Level 3 in Care is desirable.
• Experience of providing guidance and applying relevant legislation.

Responsibilities

• Develop and implement tailored support plans to improve wellbeing.
• Provide information, equipment, or independent living skills.
• Visit service users at home and carry out risk assessments.
• Respond to safeguarding enquiries and work with a multidisciplinary team.
• Ensure compliance with policies and legislation.
• Participate in an out of hours duty system.
• Travel independently across the County to visit locations.
