Job summary

Housing 21 is seeking an Assistant Care Manager to lead day-to-day delivery of care services within a designated Extra Care scheme. You will create personalised care plans, ensure safe, high-quality support, and enable residents to live with dignity and autonomy.

You will oversee rotas, monitor performance, manage risks and act as the first point of contact for residents, families and partner agencies, ensuring needs are addressed promptly.

Qualifications

• Proven management/supervisory experience in care services
• Strong understanding of care planning and safeguarding
• Strong understanding of regulatory requirements, including CQC standards
• Experience managing and scheduling staff rotas
• Excellent organisation and problem-solving skills
• Level 4 Lead Practitioner in Adult Care or equivalent or willingness to work towards

Responsibilities

• Manage day-to-day delivery of care services within a designated Extra Care scheme
• Create and regularly review personalised care plans with residents and families
• Oversee rotas and resource allocation for shifts with value for money
• Monitor performance and quality of care services, identify and manage risks
• Act as first point of contact for residents, families and partner agencies
