Job summary

West Sussex County Council is recruiting an Assistant Care Manager at Worthing General Hospital to support discharge planning and patient-centred care across the acute ward, A&E and community settings.

You will assess needs, plan and monitor care packages, and maintain accurate records, with regular supervision and some flexible hours across the county. The role offers a stable permanent contract, 37 hours per week, with a salary in the £30k band.

Qualifications

• GCSE English and Maths or equivalent literacy/numeracy level.
• Experience of providing support/working with people in a social care environment.
• Experience forming effective working relationships within a multi-agency setting.
• Awareness of policies, practices and procedures relevant to Social Work; experience with care package assessment/ Review.
• Experience applying relevant legislation, policies and procedures to individual work.

Responsibilities

• Assess and interpret information to prioritise needs and make recommendations.
• Work across a multi-agency environment to deliver a service.
• Provide guidance and communicate with a range of audiences; maintain accurate records.
• Create and monitor care plans, implement and review packages of care.
• Travel across the county and be flexible to meet customer needs; supervision provided.
