Job summary

Turning Point is seeking an Advanced Recovery Practitioner to support the RECONNECT team across Leicestershire, coordinating care for people leaving custody and guiding them into community-based services. You will collaborate with RECONNECT, conduct assessments, and ensure continuity of care post-release.

Essentials include strong report writing, experience in substance use treatment, risk management, and broad health and social care expertise.

Qualifications

• Exceptional report writing and case notes.
• Experience in substance use treatment and recovery settings.
• Understanding of risk and risk management in complex cases.
• Significant experience in health and social care.

Responsibilities

• Co-locate with the RECONNECT team at County Hall and manage a caseload.
• Engage service users in structured treatment and support resettlement.
• Visit prisons to assess needs on release and ensure continuity of care.
• Liaise with multi-agencies to signpost and coordinate support.
• Attend and contribute to meetings, sharing information on treatment progress.
• Advise RECONNECT colleagues on substance use considerations and transfer cases as needed.
• Prison vetting and travel to prisons/home visits are required (driving licence is essential).
