Job summary

Southern Health And Social Care Trust invites applications for an Advanced Physiotherapy Practitioner Level 1. The post is based in Armagh with a trust-wide footprint and will focus on early frailty intervention through virtual monitoring, education and bespoke programmes for adults over 55.

The role requires HCPC registration, a BSc in Physiotherapy and senior clinical experience in elderly/neurological rehab.

Qualifications

• HCPC registration and qualification in Physiotherapy.
• Five years post-registration experience, including senior level in elderly or neurological rehab.
• Completion of postgraduate courses in neurological/ortho-geriatric rehab.

Responsibilities

• Perform advanced clinical assessments and develop holistic management plans.
• Lead the Early Frailty Intervention Team and coordinate multi-disciplinary care.
• Provide specialist advice and education to staff and partners.
