Job summary
NHS Greater Glasgow and Clyde is seeking an experienced Advanced Frailty Practitioner to join the frailty service in Inverclyde. The role focuses on advanced assessment, rehabilitation planning and care coordination for frail older adults with complex needs.

Qualifications
- Registered nurse or allied health professional with frailty experience.
- Master's level qualification in Advanced Practice or actively pursuing it.
- UK driving licence is essential for the role; access to a vehicle for work.
Responsibilities
- Provide advanced clinical assessment, decision making and rehabilitation planning for frail older adults.
- Deliver CGA and care coordination across health and social care services.
- Support admission avoidance initiatives and promote integrated frailty pathways.
- Lead education, governance, and service development within the frailty hub and spoke model.
