University Hospitals Birmingham NHS Foundation Trust invites applications for a Band 6 Physiotherapist in the OPAL team at Good Hope Hospital, Sutton. You will deliver specialist assessment and treatment for frailty, helping to avoid admission and support discharge planning.

You will work within a seven-day, multidisciplinary team, developing expertise in frailty, acute care, and timely discharge, with opportunities to contribute to FSDEC and Home First aims.

Formación

• Registered with HCPC and physiotherapy degree.
• Experience with elderly/frailty care and multidisciplinary teams.
• CPD including recent post-graduate training relevant to clinical speciality.
• Excellent communication and evidence-based practice.

Responsabilidades

• Assess, treat and support patients throughout their journey in frailty and acute care.
• Work autonomously, lead the MDT and supervise junior staff and students.
• Coordinate onward referrals and discharge planning with community services.
• Contribute to service development and FSDEC expansion initiatives.
