NHS Greater Glasgow and Clyde is seeking an Advanced Frailty Practitioner to join the Home First Response Service. You will assess frailty in older adults, support decision-making and care planning, and help prevent unnecessary hospital admissions by coordinating safe discharges.

The role offers opportunities to work across acute, community and social care settings with a focus on person-centred care and ongoing service development.

Qualifications

• Postgraduate qualification in advanced clinical practice, frailty or clinical assessment.
• Registered healthcare professional with appropriate registration.
• Substantial post-registration experience.
• Experience with frailty and complex needs in older adults.

Responsabilités

• Undertake comprehensive frailty assessments for older adults.
• Support clinical decision-making and management planning.
• Facilitate safe discharge and admission avoidance where appropriate.
• Provide leadership within the multidisciplinary team and contribute to service development.
