Job summary
NHS Greater Glasgow and Clyde invites applications for Advanced Frailty Practitioners to join the Home First Response Service. Based at Queen Elizabeth University Hospital, you will assess frailty, guide clinical decisions and enable safe discharge while avoiding unnecessary admissions.
Join a multidisciplinary team delivering person-centred care to older adults across acute and community interfaces, with visa sponsorship considered where eligible and flexible working options where possible.
Qualifications
- Postgraduate qualification in advanced clinical practice, frailty or clinical assessment.
- Registered healthcare professional with appropriate registration.
- Substantial post-registration experience with older people.
- Strong assessment, decision-making and team-working skills.
Responsibilities
- Undertake comprehensive frailty assessments for older adults.
- Support clinical decision-making and management planning.
- Facilitate safe discharge and admission avoidance where appropriate.
- Coordinate referrals across health and social care and provide clinical leadership.
- Contribute to service development and quality improvement.
