Job summary

Buckinghamshire Healthcare NHS Trust is expanding its multidisciplinary frailty team to deliver care in Proactive Frailty Care Planning Clinics across six Integrated Neighbour Team locations in Buckinghamshire.

The Physiotherapist will conduct CGAs on moderately frail patients and work within an MDT, autonomously assessing therapy needs and providing required solutions to help patients lead healthier, more independent lives.

Qualifications

• Experience in community frailty care is advantageous.

Responsibilities

• Conduct Comprehensive Geriatric Assessments (CGAs) for moderately frail patients.
• Autonomously assess therapy needs and provide appropriate treatment plans.
• Deliver care in Proactive Frailty Care Planning Clinics as part of an MDT.
• Support patients to lead healthier, more independent lives.
