Job summary

Widnes Highfield Health Ltd is seeking an Advanced Clinical Practitioner to join the Widnes Primary Care Network and deliver an innovative frailty service across GP practices, patients' homes and care homes. You will perform comprehensive geriatric assessments, coordinate care with a multidisciplinary team and promote anticipatory planning to improve outcomes and reduce avoidable hospital admissions.

Qualifications

• Experience working with older people and/or patients living with frailty.
• Experience undertaking holistic patient assessments.
• Experience managing patients with multiple long-term conditions.
• Experience of multidisciplinary working.
• Experience of care planning and care coordination.

Responsibilities

• Deliver patient-centred nursing care for frailty across GP practices, homes and care homes.
• Undertake CGAs and contribute to personalised care planning.
• Develop and review anticipatory care plans with patients, carers and MDT.
• Monitor frailty patients and escalate concerns appropriately.
• Identify and manage common frailty syndromes (falls, delirium, immobility, continence, malnutrition, medications).
• Collaborate with dietitians, pharmacists and other professionals for nutrition optimisation.
• Support structured medication reviews with the pharmacy team.
• Carry out cognitive assessments and refer to memory services as needed.
• Identify patients benefiting from palliative approaches and facilitate referrals.
• Support ReSPECT discussions and documentation of patient wishes.
