Job summary

HCRG Care Group is seeking an experienced Advanced Nurse Specialist to lead CHC services for adults with learning disabilities. You will coordinate complex assessments, supervise CHC nurses and drive person-centred care within a multidisciplinary framework.

The role offers hybrid working across Wiltshire and surrounding areas, with an emphasis on clinical leadership, service development and improving outcomes for people with learning disabilities.

Qualifications

• Current NMC registration as a Registered Nurse.
• Significant post-registration experience working with people with learning disabilities and complex needs.
• Extensive knowledge of the NHS Continuing Healthcare (CHC) National Framework.
• Experience completing CHC Checklists, DSTs, reviews and complex eligibility assessments.

Responsibilities

• Lead and support a dedicated team of Continuing Healthcare Nurses, providing clinical guidance and professional support.
• Coordinate and oversee complex CHC assessments, reviews and eligibility processes.
• Manage complex caseloads to optimise health and care outcomes for individuals with learning disabilities.
• Build relationships with families, carers and partner organisations to deliver person-centred care.
• Develop team members and promote professional growth.
• Participate in multidisciplinary meetings and safeguarding processes.
• Ensure care packages remain effective, safe and responsive to changing needs.
• Contribute to service improvement initiatives that enhance outcomes and experiences for service users.
