Job summary

HCRG Care Group is seeking an Advanced Clinical Practitioner to join the Hospital at Home service, delivering acute hospital-level care across Wiltshire and neighbouring localities from a North Wiltshire base.

You will assess, diagnose and manage complex patients in home settings, provide clinical leadership within multidisciplinary teams, and help reduce hospital admissions while supporting safe discharges. NHS Pension and ongoing professional development available.

Qualifications

• Registered with the NMC or HCPC.
• Accredited Advanced Clinical Practice qualification.
• Non-medical prescriber qualification.
• Significant experience in frailty, elderly care, community services or acute care interfaces.
• Proven experience managing complex clinical caseloads and working autonomously at an advanced level.
• Demonstrable leadership and supervision experience within multidisciplinary teams.
• Clean driving licence and access to a vehicle for daily use.

Responsibilities

• Deliver a clinically autonomous service across Urgent Crisis Response, admission avoidance, frailty and Virtual Ward pathways.
• Assess, diagnose and manage patients with acute and complex needs, enabling safe discharge.
• Provide clinical leadership within the multidisciplinary team and role model best practice.
