We are looking for a Advanced Nurse Practitioner - Prescriber to join our forward thinking and dynamic team.You will be working in our busy GP practice in the Castleford area with a list size of over 11,000 patients.

Main duties of the job

You will help to deliver services to our local population.

We are looking to recruit an Advanced Nurse Practitioner - Prescriber.

Fulltime of 37.5 hours per week.

Henry Moore Clinic has 2 sites with 5 GP Partners with over 11,000 patients.

Our practice has a very strong training ethos. We train GP Registrars and medical students and on occasion it may be a requirment that a trainee spend time with you in clinic.

As a forward-thinking practice, we require a similarly motivated and enthusiastic Practice Nurse to join our friendly and supportive team.

Job responsibilities

The Advanced Nurse Practitioner will work as part of theprimary healthcare team providing Personal Medical Services to the patients ofpractice.

Job responsibilities:

To work autonomously, the ANP is responsible for consultingwith patients who wish to be seen by a health care professional; diagnosing andtreating patients presenting with minor illness; seeing and advising people inrespectoftheir continuing medical and nursing needs. All consultations willbe recorded on SystmOne, the clinical IT system. The ANP is required to conductboth telephone and F2F appointments.Intermittently there is a requirement to see extra or emergency patients andcarry out home visits for an agreed cohort of patients. The nurse practitionerwill need to prioritise and triage the needs of patients accordingly, makingany necessary referrals for investigations in a timely appropriate manner.Patients with conditions requiring referral to secondary care are discussedwith the registered GP before making such a referral.

Diagnosing and treating patients

Triage and treat patients presenting with minor illnesses tosee a health care professional, making any necessary referrals to other membersof the primary health care team.

Ensure clinical practice is safe and effective and remainswithin boundaries of competence and to acknowledge individual limitations.

Advise patients on general health care and minor ailmentswith referral to other members of the primary and secondary health care team asnecessary.

Undertake the collection of pathological specimens includingintravenous blood samples, swabs etc. Perform appropriate investigatoryprocedures requested by the GPs.

Administration and profession responsibilities

Participate in the administrative and professionalresponsibilities of the practice team.

All patient activities are completed daily.

Ensure accurate and legible notes of all consultations andmanagement plans are recorded in the patients notes on the clinical system,SystmOne.

Ensure accurate completion of all necessary documentationassociated with patient health care and registration with the practice.

Ensure collection and maintenance of statistical informationrequired for regular and ad hoc reports and audit.

Attend and participate in practice meetings and mandatorytraining sessions.

Undertake the online training and other training pertinentto the role of ANP as agreed with the Practice Manager/Partners.

Health & Safety:

The post-holder will participate in a full range ofpromotion and management of their own and others health and safety andinfection control, as defined in the practice Health & Safety policy, thepractice Health & Safety manual, and the practice Infection Control policyand published procedures. This will include (but will not be limited to):

Using personal security systems within the workplaceaccording to practice guidelines.

Awareness of national standards of infection control andcleanliness and regulatory / contractual / professional requirements, and goodpractice guidelines.

Responsible for correct and safe management of the specimensprocess, including collection, labelling, handling, use of correct and cleancontainers, storage and transport arrangements.

Ownership of infection control and clinically based patientcare protocols and implementation of those protocols across the practice.

Active observation of current working practices across thepractice in relation to infection control, cleanliness and related activitiesensuring that procedures are followed and weakness / training needs areidentified, escalating issues as appropriate.

Identifying the risks involved in work activities andundertaking such activities in a way that manages those risks across clinicaland patient process.

Making effective use of training to update knowledge andskills, and initiate and manage the training of others across the full range ofinfection control and patient processes.

Monitoring practice facilities and equipment in relation toinfection control along with other members of the nursing team, ensuring thatprovision of hand-cleansing facilities, wipes etc. are sufficient to ensure agood clinical working environment. Lack of facilities to be escalated asappropriate.

Safe management of sharps procedures, including training,use, storage and disposal.

Using appropriate infection control procedures, maintainingwork areas in a tidy, clean and sterile, and safe way, free from hazards.Initiation of remedial / corrective action where needed or escalation toresponsible management.

Actively identifying, reporting, and correction of healthand safety hazards and infection hazards immediately when recognized.

Keeping own work areas and general / patient areas generallyclean, sterile, identifying issues and hazards / risks in relation to otherwork areas within the business, and assuming responsibility in the maintenanceof general standards of cleanliness across the business in consultation (whereappropriate) with other sector managers.

Undertaking periodic infection control training.

Waste Management, including collection, handling,segregation, container management, storage and collection.

Decontamination control procedures, management and training,and equipment maintenance.

Maintenance of sterile environments.

Demonstrate due regard for safeguarding and promoting thewelfare of children.
