Practice Plus Group in the UK is recruiting an Advanced Nurse / Clinical Practitioner for a remote, telephone and digital triage role in South West London. You will join a multidisciplinary team delivering timely clinical advice and guiding patients to the right care pathway.

This home-based role emphasizes flexible, accessible care and reducing unnecessary hospital visits, with training and development through a bespoke Learning Management System.

You’ll help ensure patients receive the right care, in the right place, at the right time reducing unnecessary hospital visits and improving outcomes across the system.
