• Registered Nurse or Therapist with advanced practitioner experience.
• Community experience with advanced clinical assessment skills.

Responsibilities

• Provide urgent health care to patients in their homes.
• Develop and shape the two-hour response service.
• Collaborate with multidisciplinary teams to ensure safe patient care.
