Calderdale and Huddersfield NHS Foundation Trust is seeking an experienced Advanced Clinical Practitioner to join the Acute Stroke Service. The ACP will perform advanced clinical assessments, diagnose, treat, and manage patients with suspected or confirmed acute stroke and TIA, within HASU, acute wards, and rapid-access clinics.

The role emphasises four pillars of practice: clinical excellence, leadership, education, and research, with a focus on direct patient care and service development.

Qualificações

• Advanced clinical assessment and autonomous decision-making in acute stroke care.

Responsabilidades

• Undertake advanced clinical assessment for patients with acute neurological deficits, including differentiation between stroke, TIA, and stroke mimics.
• Provide expert clinical decision-making in complex, time-critical situations.
• Hold direct responsibility for a caseload of acute stroke and TIA patients with evidence-based care plans and escalation when required.
• Support hyper-acute stroke pathways including initial assessment, prioritisation, urgent imaging, thrombolysis and thrombectomy pathways, and ongoing monitoring.
