Dorset HealthCare University NHS Foundation Trust is seeking an Advanced Clinical Practitioner to join the Bridport Hospital team. The role is full-time (37.5 hours per week) and requires autonomous clinical decision-making with a focus on older persons inpatient medical and rehab, including stroke neuro rehabilitation.

The post includes leadership and expert clinical advice, with access to ACP networks and ongoing development.

Responsibilities

• Plan and manage complete episodes of care.
• Undertake independent comprehensive assessments and formulate management plans.
• Communicate with acute hospitals, primary and social care teams to ensure seamless continuity of care.
• Undertake audits and service evaluations to inform service improvement.
