The Rehab Care Coordinator at Northern Montana Hospital supports the physical, occupational and speech therapy teams by identifying patient equipment needs, coordinating care logistics, and auditing documentation to ensure timely, quality care.

This role also assists the Rehab Services Director with quality assurance and standards compliance, arranges adaptive equipment and supplies, and helps educate patients and families, taking into account age, gender and cultural differences.

Qualifications

• Graduation from an AOTA approved Certified Occupational Therapy Assistant educational program and successful completion of the field work requirements.
• Licensed to practice in the state of Montana.
• BLS certification required.
• Minimum of 2 years experience in rehab field strongly preferred.
• Ability to communicate effectively and work well with other healthcare providers.
• Ability to work independently and creatively.
• Interest in continuing to learn and grow professionally.
• Ability to hold in confidence all personal, hospital, and professional matters.

About Northern Montana Hospital

About the role

The Rehab Care Coordinator supports the physical, occupational and speech therapy teams by identifying and coordinating patient medical equipment needs, managing patient care logistics, conducting documentation audits, facilitating community programming, and assisting the Rehab Services Director in achieving quality assurance and standards compliance. This role is essential in ensuring patients receive timely and effective care, from scheduling and documentation to securing necessary resources and equipment. Rehab Care Coordinator duties include carrying out follow‑up treatments, facilitate positioning and adaptive equipment, and assist with other duties, including equipment and supply organization and maintenance as assigned by the OTR or Rehab Services Director taking into consideration patient needs based on gender, age, cultural differences, and educational needs.

Qualifications
