The role offers a permanent contract, 24 hours weekly, and a pro rata salary in the £27,709–£29,071 range. A 10% night supplement is paid when requirements are met.

Norfolk Swift Response is looking for Reablement Liaison Officers to join our dedicated team-providing essential coordination and support that helps vulnerable people. What You'll Be Doing As a Reablement Liaison Officer, you'll be the first point of contact for referrals and queries, working closely with GPs, community health teams, social services, service users and their families. You'll be part of a supportive and friendly team and your responsibilities will include:
You’ll be at the heart of a service that operates 24/7, 365 days a year - helping people remain at home and avoid unnecessary hospital visits. Your coordination and decision-making will directly impact the quality and speed of care delivered.
This is a hybrid role, with shifts split between home and office. Your office base will be County Hall in Norwich or other NCC offices as required. Shifts are 9.45pm to 6.15am, including weekends and bank holidays on a two-week rota (pattern to be discussed at interview). A 10% supplement for night roles is paid when all requirements are met.
