East Suffolk and North Essex NHS Foundation Trust is seeking a dedicated Care Co-ordination Centre administrator to support patient care. The role involves handling communications, data entry into IT systems, and coordinating referrals and appointments with clinical staff.

The Care Coordination Centre (CCC) is the single point of access for patients that need care in the community. Operational 24 hours per day, 7 days per week we are the referral route for both East and West Suffolk based patients. The Care Coordination Centre has established itself as the starting point for many specialist services as their dedicated referral route and is expanding all the time.
