Job summary
A community support organization in Toronto is seeking a Temporary Support Worker to enhance the capacity of individuals in community living. The role involves identifying needs, providing personal care, and implementing life plans. Candidates should have a post-secondary education in Social Services and knowledge of related health-care procedures. This position offers 37.5 hours of work per week.
Qualifications
- Post-secondary education focused on Social Services or Developmental Services.
- Knowledge of health-care procedures and mechanical equipment.
- Successful completion of internal training.
Responsibilities
- Identify program needs of individuals supported and implement life plans.
- Provide personal and physical care, medication administration, and advocacy.
- Facilitate life-skills training and emotional support for individuals.

Temporary Support Worker 3 (position duration of up to six months)
Supported Living West
37.5 hours per week
Identifies the needs of individuals supported and delivers supports and programs consistent with life plans in order to enhance their capacity to function in all aspects of community living.
- Under the direction of a program supervisor, identify program needs of individuals supported, participate in the development and implementation of life plans, and assist in monitoring achievement of personal outcomes.
- Provide case management to individuals on caseload, including appropriate personal and physical care, medication administration, crisis response, advocacy and liaison with families, medical and other professionals, service providers to address needs.
- Devising and providing life‑skills training and emotional support to individuals supported on a one‑on‑one or group basis, and refer to professionals when necessary.
- Maintain documentation, monitor finances, ensure an appropriate standard of living and assist with locating housing for individuals supported.
- Provide direction to part‑time staff as required.
- Post‑secondary education with a focus on Social Services or Developmental Services, and knowledge of health‑care procedures and mechanical equipment (wheelchairs, lifts, G‑tubes and other specialized equipment).
- Post‑secondary education in a non‑related field will require an additional minimum of 1 year of related work experience.
- Successful completion of internal legislated and mandated training.
- Valid driver’s license is an asset.
