A community mental health organization in Oshawa is seeking a Nurse Practitioner to deliver primary care and support mental health services. The role emphasizes health promotion, patient education, and disease prevention in a collaborative, multidisciplinary environment. The ideal candidate has a Master's in nursing, is registered with the College of Nurses of Ontario, and possesses excellent communication and assessment skills. This position is located in a dynamic treatment environment, with flexibility required for various hours.
CMHA Durham is an integrated community mental health and primary care hub that has been providing mental health services to Durham Region for over 60 years. The organization offers a broad range of services through inter-professional teams focused on clinically based services that address social determinants of health using the recovery model.
The Nurse Practitioner provides services to complex clients throughout the lifespan, the client family, and the community, with a strong emphasis on health promotion, patient education and disease prevention in a psychiatric and addictions treatment setting. The role includes participation as a member of a high-performance, multi-service team of Case Managers, nurses and other health specialists providing Primary Care to clients within the HART Hub residential setting.
Adhere to CMHA Durham client safety programs and infection prevention procedures. Maintain open communication with clients, families and staff to achieve quality safety outcomes. Report client safety incidents and suggest improvements to management.
