Job summary

WoodGreen Community Services is seeking a Client Support Worker for the Homes for Good (St. Hilda’s) program.

This part-time role focuses on supporting seniors experiencing homelessness with mental health or substance use challenges, through on-site services and 24-hour staffing support when needed. You will assist with daily living, care planning, and connections to community resources, while maintaining documentation and ensuring safety.

Qualifications

• 2+ years in supporting older adults or vulnerable populations.
• Experience working with homelessness, mental health, or substance use is preferred.

Responsibilities

• Build positive, respectful relationships with program participants and provide emotional support.
• Deliver one-on-one support aligned with care plans and assist with form completion and navigation of resources.
• Participate in intake processes including screening, interviews, onboarding, and follow-up for new participants.
• Monitor client progress and well-being, contribute to care plan evaluations, and suggest adjustments.
• Support goal setting and care planning to promote independence and community integration.
