Job summary

WoodGreen Community Services is seeking a part-time Client Support Worker for the Cummer Housing Program in Toronto. The role focuses on assisting seniors experiencing homelessness with goals, daily living, and access to health and social services, in a supportive onsite setting.

You will build relationships, support care planning, manage intake, and help navigate housing resources, while promoting independence and community integration.

Qualifications

• Social Service Worker diploma or related education.
• 2+ years’ experience supporting older adults with homelessness, mental health concerns, or substance use.
• Ability to interact with high‑risk populations and use de‑escalation techniques.

Responsibilities

• Build positive, respectful relationships with participants and provide emotional support.
• Deliver one‑on‑one support aligned with care plans and assist with form completion.
• Participate in intake, screening, onboarding and follow‑up for new participants.
• Monitor progress and well‑being, contributing to care plan evaluations.
• Assist with activities of daily living and life skills training.
• Coordinate housing stabilization, waitlists, and liaison with housing supports.
• Encourage participation in community activities and connect to services.
• Collaborate with external providers to strengthen support networks.
• Respond to crises using de‑escalation and trauma‑informed approaches.
• Document case notes timely and maintain health and safety standards.
