Job summary

WoodGreen Community Services is seeking a Part Time Client Support Worker for the Cummer Housing Program, on-site in Toronto. You will support seniors facing homelessness with housing, health, and social services, promoting independence and well-being.

You will build relationships, assist with care plans, crisis support, ADL/IADL assessments, and connect clients to community resources. Strong communication and de-escalation skills are essential, with 2+ years of relevant experience preferred.

Qualifications

• Social Service Worker diploma or related education.
• 2+ years’ experience supporting older adults facing homelessness, mental health, or substance use challenges.

Responsibilities

• Build positive, respectful relationships with participants and encourage engagement.
• Deliver one-on-one support aligned with care plans, assist with forms and resources.
• Participate in intake, screening, onboarding and participant follow-up.
• Monitor progress and well-being, update care plans as needed.
• Support goal setting and care planning to promote independence and community integration.
• Assist with activities of daily living and use ADL/IADL tools to assess needs.
• Facilitate housing stabilization by helping with applications and waitlists.
• Encourage participation in community activities and connect with services.
• Collaborate with external providers to strengthen support networks.
• Respond to crises using de-escalation and trauma-informed approaches.
• Perform health-related tasks under professional guidance, including medication management.
• Arrange and accompany clients to appointments and activities when needed.
• Maintain accurate case notes and documentation in a timely manner.
• Uphold health and safety standards and emergency procedures.
