Job summary

WoodGreen Community Services is seeking a Client Support Worker for the Homes for Good program, providing compassionate support to seniors facing homelessness and challenges related to mental health and substance use. This role is on-site at 800 Vaughan Road, Toronto, and requires collaboration with a multidisciplinary team to promote client well-being and independence.

Qualifications

• Social Service Worker diploma or related education.
• 2+ years’ experience supporting older adults with homelessness, mental health concerns, and/or substance use.
• Ability to interact effectively with high-risk, marginalized individuals.
• Demonstrated de-escalation and boundary-setting skills.
• Strong written, oral, and interpersonal communication skills.
• Ability to work independently and as part of a team.
• Knowledge of community resources in Toronto.

Responsibilities

• Build positive relationships with program participants and provide emotional support.
• Deliver one-on-one support aligned with care plans and navigate resources.
• Participate in intake processes including screening, interviews, onboarding and follow-up.
• Monitor client progress and contribute to care plan evaluations.
• Assist with activities of daily living and documentation.
