Job summary

Indwell is seeking a part-time Supportive Housing Coordinator to work with residents facing mental health challenges and/or substance use. You will help develop and implement individual service plans, coordinating with an interdisciplinary team to promote health, wellness, and housing stability.

The role requires resilience and comfort working in challenging situations, with a commitment to Indwell’s values of dignity, love and hope.

Qualifications

• Post-secondary education in a related field or equivalent.
• Experience working within a case management framework with individuals with complex needs.
• Strong interpersonal and communication skills to support residents and collaborate with a team.

Responsibilities

• Provide direct support to residents, developing individual support plans.
• Coordinate and participate in case conferences with external supports.
• Utilize a trauma-informed and harm-reduction approach to promote stability.
• Assist in intake, orientation, and group programming.
• Maintain thorough documentation and protect tenant health records.
