Job summary

Indwell is seeking a Registered Social Worker to join a multidisciplinary team in St. Thomas, Ontario. The role focuses on coordinating and delivering mental health and addiction services to tenants, with an emphasis on transition care and housing stability.

You will screen and assess residents, develop care plans, and collaborate with partners to support long-term wellbeing. Strong leadership, cultural safety, and trauma‑informed practice are valued.

Qualifications

  • Bachelor of Social Work (BSW) degree or equivalent.
  • Registered with a regulated college of professionals.
  • Recent Police Vulnerable Sector Check (PVSC).
  • Current Standard First Aid & CPR Certification.
  • Experience working with individuals with serious mental illness, addictions, or other special needs.
  • Knowledge of concurrent disorders and addiction treatments/theories.
  • Warm, sensitive, and caring approach with teamwork and initiative.
  • Strong problem-solving skills within a supportive team.
  • Harm reduction and trauma-informed lens in care.
  • Experience supporting diverse individuals and communities (cultural safety, anti-racist and anti-oppressive practice).
  • Strong leadership and mentoring abilities.
  • Valid G-class driver’s license with own vehicle.

Responsibilities

Position Description: Reporting to theProgram Supervisor, as the Registered Social Worker, you will be working within the context of a collaborative interdisciplinary team reflecting the principles of person-centered and evidence-based care by assisting with coordinating and delivering mental health and addiction services to tenants.You will be supporting these individuals who have experienced long term housing instability and/or long stays in hospitals or in other institutional settings.The focus will be on specificallyassistingthem in times of transition to/from hospital, incarceration,treatmentand intake intoIndwell’sprogram to help them achieve health and housing stability.This position will requirea high levelof resilience and a willingness to work near challenging situations.

Hours of Work: Full time 40 hours per week, 8 hours a day. Schedule based on the support needs ofindividuals in the program, includingsomeevenings,weekends and public holidays.Positionmayrequire on call hours.

WHAT YOU WILL BE DOING:
Directly Supporting Tenants
  • Skilled in building therapeutic relationships withresidents
  • Trauma-informed approach promoting hope and trust whilemaintainingappropriate professionalrelationships
  • Program screening, intake, assessments, and referrals
  • Work individually withresidentstoidentifyand achieve substance related goals as it relates to their individual support needs and recovery
  • Work collaboratively with internal and external partners
  • Supportresidentsin issues related tomaintainingtheir health and housing
  • Assess complex situations and makeappropriate decisionsindependently and as part of a team
  • Confident inutilizingcrisis management and intervention skills
  • Develop andfacilitaterelevant group programming to meet the needs of the community
  • Knowledge of community resources and provincial addiction treatment service
Support, Direction, and Commitment to Interdisciplinary Team
  • Partner with other program staff from multiple disciplines in a participatory, collaborative, and coordinated approach to shared decision-making around case management and referrals
  • Medication administration, apartment checks, community events, and other supports as necessary
  • Work within a team in on-site, in-home assessments and interventions to support long-term stable housing through effective holistic supports
  • Utilizing various asset-based and person-centered frameworks to create and manage care plans with tenants
  • Enablingresidentsto build capacity to self-manage their health and related concerns, connectresidentsto appropriate community and social service supportsfor health promotion and maintenance, and educateresidentson system navigation and self-advocacy skills and resources within healthcare and community and social services
  • Compiling up-to-date resources on community and social servicesfacilitatinginformation sessions and workshops for tenants on various public health and community and social service topics
  • Familiarity and comfort advocating and educating within the mental health system.
Assessment and Documentation
  • Participate intheIntake process and assessments
  • Lead the development of coordinated care planning with internal and external supports
  • Complete required documentation in a thorough andtimelymanner with a focus onmaintainingand protectingresidenthealth records
  • Perform administrative tasks as assigned, such as rent collections and financial records
  • Intake
  • Collect initial information todetermineeligibility and ensure applicants are appropriately matched to services.
  • Assessment
  • Gather holistic information about theresident’shealth, wellness, social belonging, strengths, risks, barriers,metand unmet needs.
  • Planning
  • Collaboratively developing an individualized, written service plan or coordinated care plan that outlines specific goals, planned interventions, responsible parties, timelines, and required resources/services.
  • Implementation & Coordination
  • Connectresidentsto needed services, coordinate care across internal and external supports, and advocate on behalf of the client when necessary.
  • Evaluation
  • Regularly review progress, reassess theresidentsstatus, and adjust the service plan as needed to ensure it remains relevant and effective.
  • If required, plan and execute safe, well-coordinated transitions, ensuring continuity of care and appropriate follow-up supports.
WHAT YOU WILL BRING:
  • Bachelor of Social Work (BSW) degree or equivalent
  • Registered with a regulated college of professionals or willing to get registered for that discipline
  • Recent Police Vulnerable Sector Check (PVSC) with the information therein acceptable by Indwell to meet the operational requirements of the program- at one’s own cost
  • Current Standard First Aid & CPR Certification from an approved provider that meets Ontario provincial standardsor willingness to register for the training within 30 days of employment-reimbursement by Indwell
  • Experience working with individuals with serious mental illness,addictions, or other special needs
  • Knowledge of concurrent disorders and of various addiction treatments/theories is required
  • A warm, sensitive, and caring approach, the ability to work within a team, be flexible, organized, responsible, and take initiative.Having strong problem-solving skills, being able to maintain good interpersonal relationships and a genuine interest in supporting people living with mental illness and addictions.
  • A harm reduction and trauma-informed lens, seeking to uphold the health and wellness of individuals as well as the community
  • Experience and ongoing learning around supporting diverse individuals and communities (i.e. cultural safety, anti-racist and anti-oppressive practice, etc.)
  • Strong leadership skills, critical thinking abilities and has a passion for mentoring others
  • A valid “G” class driver’s licensewith own vehicle
