Bayaud Works, LLC is seeking a Case Manager to provide trauma-informed, housing-focused services for families in the Tamarac Family Emergency Non-Congregate Shelter. You will identify housing barriers, develop stability plans, coordinate wraparound services, and connect residents with resources to improve housing stability, income, health, and self-sufficiency.

Qualificações

• Associates degree in social work or related field required or equivalent experience.
• Minimum 2 years’ case management experience preferred.
• Experience with HMIS or similar data systems preferred.
• Experience working with families experiencing homelessness preferred.
• Bilingual Spanish/English preferred.

Responsabilidades

• Provide housing-focused case management, including intake, assessment, planning, referrals, follow-up and stabilization.
• Assess resident needs, barriers to housing, income supports, benefits eligibility and stability factors.
• Develop individualized housing stability and employment plans with goals and timelines.
• Support housing navigation, landlord communication, documentation, and community resources.
• Apply trauma-informed, culturally responsive practices to promote dignity and autonomy.
• Maintain accurate documentation in HMIS and shelter systems per standards.
• Participate in team meetings, case conferencing, and coordination with partners.
