Job summary

Bissell Centre is seeking a Case Manager to join the Hope Terrace team. You will build and navigate client-directed care plans, assess psychosocial needs, and collaborate with the care team to support residents’ wellbeing.

The role emphasizes trauma-informed, client-centered practice and strong relationship-building to help residents access services and improve outcomes. Responsibilities include developing individualized care plans, conducting psychosocial assessments, and advocating for

Qualifications

• Minimum degree or diploma in Human Services.
• 2–3 years direct experience working with vulnerable populations.
• Active RSW license in Alberta preferred and in good standing.

Responsibilities

• Develop an individual care plan for each resident using Hope Terrace care pathways.
• Provide ongoing supportive case management to residents.
• Initiate and complete ongoing needs assessments, referrals, and advocacy for individuals in the Hope Terrace program.
• Complete comprehensive psychosocial assessments as required.
• Identify opportunities to assess and enhance readiness to make positive changes and increase self‑reliance related to mental health and substance use concerns.
