Job summary

Bissell Centre is seeking a dedicated Case Manager to join our team at Terrace Heights Supportive Housing in Edmonton. You will deliver individualized case management to residents, coordinating supports to promote housing stability, wellness, independence, and community integration.

Reporting to the Manager, Supportive Housing, you will assess needs, develop recovery plans, advocate for services, and work with our interdisciplinary team to help residents achieve meaningful recovery and

Qualifications

  • Degree or diploma in Social Work, Human Services, Addictions and Mental Health, or a related field.
  • Minimum of two to three years of direct case management experience with individuals facing complex needs, homelessness, mental health concerns, or related barriers.
  • Experience in supportive housing, residential or recovery-oriented environments is an asset.

  • Provide comprehensive, individualized case management to residents to promote housing stability, wellness, independence, and community connection.
  • Collaborate with residents to identify goals and develop person-centred care and recovery plans.
  • Coordinate services, advocate, provide referrals, and connect residents to health, mental health, income, and social supports.
  • Assess residents’ needs, monitor progress, and support recovery, wellness, and independent living goals.
  • Utilize trauma-informed, recovery-oriented, and culturally responsive approaches to support engagement and positive change.

How You’ll Make an Impact?

The Case Manager, under the direction of the Manager, Supportive Housing, provides comprehensive, individualized case management to residents of Terrace Heights Supportive Housing. The Case Manager works collaboratively with residents to identify strengths, needs, goals, and barriers and to develop person-centred care and recovery plans that promote housing stability, wellness, independence, community connection, and meaningful recovery.

Using trauma-informed, recovery-oriented, and culturally responsive approaches, the Case Manager provides ongoing assessment, service coordination, advocacy, referrals, and connection to health, mental health, substance use, income, cultural, recovery, and community-based supports. The Case Manager works closely with Residential Support Workers, the interdisciplinary care team, and external service providers to coordinate supports, monitor progress, respond to changing needs, and support residents in achieving their individualized goals and greatest possible level of independence and well-being.

  • Provide comprehensive, individualized case management that supports residents in achieving housing stability, wellness, recovery, independence, and improved quality of life.
  • Complete comprehensive psychosocial and ongoing needs assessments to identify resident strengths, needs, barriers, risks, and opportunities for growth and recovery.
  • Collaboratively develop, implement, monitor, and regularly review individualized care and recovery plans based on each resident’s identified goals, strengths, needs, and stage of change.
  • Support residents to identify meaningful and achievable goals related to housing, physical and mental health, recovery, income, relationships, cultural connection, daily living, and community participation.
  • Utilize motivational, strengths-based, trauma-informed, person-centered, and recovery-oriented approaches to support resident engagement and positive change.
What You’ll Bring?
  • Degree or diploma in Social Work, Human Services, Addictions and Mental Health, or a related field.
  • Minimum of two to three years of direct case management experience working with individuals experiencing complex needs, homelessness, mental health concerns, substance use, or other barriers to housing stability.
  • Experience working within supportive housing, residential, community-based, or recovery-oriented environments is considered an asset.
  • Experience completing assessments, developing individualized care and recovery plans, coordinating services, providing advocacy, and navigating health and social-service systems.
  • Experience supporting individuals in identifying and working toward recovery, wellness, and independent living goals using person-centered, and trauma-informed approaches.
