Job description

Job Title: Team Leader, Correctional Facility

The Team Leader, Correctional Facility, is part of the program's management structure. The role involves supervising a team to ensure quality service delivery to a specific patient population at the county jail, acting as the senior clinician or "player coach." This position reports to the Area Director, Clinical Operations.

Essential Duties & Responsibilities
  1. Lead and monitor the team daily, ensuring services are provided to patients at McHenry County Jail awaiting placement at the state hospital for fitness restoration.
  2. Review referrals from the Department of Mental Health (DMH), assess patients, and collaborate on treatment plans with prescribers and stakeholders.
  3. Oversee staff activities, including care coordination, case management, and patient support services.
  4. Collect data, complete quarterly reports for DMH, and ensure timely documentation and confidentiality of case records.
  5. Manage referrals, discharges, and transitions in coordination with DMH, correctional staff, and medical teams.
  6. Participate in hiring, evaluations, and staff development, including conducting annual reviews and setting professional goals.
  7. Perform other duties as assigned, including representing the agency in meetings and staff training.
  8. Schedule: Monday-Friday, 8:30 am - 5:00 pm.
Marginal Duties & Responsibilities
  • Work flexible hours, including weekends or evenings as needed.
Qualifications
  • Master's degree in social work, counseling, psychology, or a related field.
  • At least two years of clinical experience.
  • Preferred: LCSW or LCPC licensure, leadership experience, proficiency with electronic medical records, and strong interpersonal skills.
  • Reliable transportation to correctional facilities is required.
