Job description

Overview

This position is responsible for providing clinical social work services to hospital patients and their families, ensuring excellent patient care within an interdisciplinary team setting. Responsibilities include comprehensive assessment and treatment planning, educational and supportive counseling, resource identification, referral, and collaboration with inter-professional team members and community-based service providers. Key workdays are Friday, Saturday, and Sunday. Coverage areas include the Emergency Department, Labour and Delivery, and NICU.

Education

• Member of the Ontario College of Social Workers & Social Service Workers or a candidate for certification.
• Master of Social Work from an accredited School of Social Work or an equivalent degree recognized by the Ontario College of Social Workers & Social Service Workers.

Qualifications

• Recent experience in Maternal, Newborn, and Pediatric environments is an asset.
• Knowledge of CAS.
• Experience working with community resources.
• Ability to develop and maintain positive relationships with social work staff, other disciplines, and community partners.
