Job summary

AgeCare is seeking a Licensed Practical Nurse (LPN) to provide goal-directed, patient-centered nursing care in a supportive environment for residents in Medicine Hat, Canada. Reporting to the Director of Care, you will join an interdisciplinary team to deliver high-quality care that preserves residents’ dignity and well-being.

You will assess and update care plans, administer medications, coordinate with physicians, and mentor unregulated care providers.

Qualifications

• Current LPN licensure in the applicable province.
• Experience in senior or long-term care is preferred.
• CPR and First Aid certification is current.
• Proficiency with electronic documentation and Microsoft Office.
• Completion of a Vulnerable Sector Check (or willingness to obtain).

Responsibilities

• Collaborate with the care team using a person-centered approach.
• Maintain resident assessments, care plans, and documentation; share updates.
• Implement and evaluate care plans; adjust as needed.
• Participate in resident care conferences and scheduling supports.
• Assist with transitions including move-ins, discharges, and transfers.
• Provide direct nursing care and administer medications per policies.
• Safely manage medications and narcotics per scope and legislation.
• Educate residents and families about conditions and care plans.
• Coordinate care plans with physicians and team members.
• Delegate tasks to unregulated care providers as appropriate.
• Oversee emergencies in the absence of senior leadership.
• Address resident/family concerns with timely follow-up.
• Support infection prevention and quality improvement initiatives.
• Uphold ethics and organizational values.
• Perform other related duties as assigned.
