Job summary

Extendicare in Canada is seeking a licensed practical nurse (LPN) to provide patient-centered nursing care in accordance with provincial standards. Reporting to the Director of Care, the LPN will collaborate with an inter-professional team to assess needs, develop and document care plans, administer medications, and respond to changes in residents' conditions.

Qualifications

• Current registration as a Licensed Practical Nurse in the applicable Province/State.
• Experience in gerontology, rehabilitation, community/long term care an asset.

Responsibilities

• Provide direct resident care and medication/treatment administration.
• Complete resident care assessments, care/service plans and documentation.
• Coordinate and communicate changes in residents' condition with the care team.
