Job summary

Quilt, Inc. is seeking compassionate caregivers for a 24-hour palliative care assignment near Dutton, Ontario. The client has stage 4 lung cancer with bone and brain metastases and requires continuous support at home on a rural farm.

Role includes 12-hour shifts in a rotating team, with meals, mobility assistance, incontinence care, transfers, and close coordination with family and healthcare professionals to ensure comfort during the end-of-life period.

Qualifications

• Personal Support Worker certification or equivalent caregiving experience.
• Experience with palliative care and end-of-life support preferred.
• Comfortable with incontinence care and mobility assistance.
• Ability to work independently in a rural farm setting.
• Reliable transportation to reach rural location 30 minutes from London.
• Compassionate, patient, and respectful approach to care.
• Flexibility to work 12-hour shifts as part of a rotating team.
• Willingness to collaborate with family members and healthcare professionals.

Responsibilities

• Provide continuous 24-hour personal care and companionship.
• Assist with incontinence care including regular changing and personal hygiene.
• Support safe transfers from bed to chair and assist with wheelchair mobility.
• Facilitate outdoor time on the back deck for fresh air when client is able.
• Prepare and assist with meals and hydration.
• Monitor client condition and report any changes or concerns to family and care coordinator.
• Coordinate with Ontario Health at Home palliative nursing team as needed.
• Maintain detailed shift reports and communication with relief caregivers.
