Sienna Senior Living is seeking an RAI Coordinator RPN at Oakwood Commons Community in Brantford, ON. The role focuses on acting as the RAI MDS 2.0 resource for the care community and ensuring accurate, timely MDS coding and care plan updates.

The successful candidate will review monthly RAI MDS for residents, manage the data submission process, and monitor outputs while collaborating with the management team. A health-profession registration and AIS certification are required.

Qualifications

• Problem solving, assessment, and analytical abilities are required.
• Current general certificate of registration with a College of a Regulated Health Profession or Ontario College of Social Workers is required.
• Successful annual completion of RAI MDS certification as per Ministry of Health requirements for AIS.
• Vulnerable Sector Check must be passed.

Responsibilities

• Act as the RAI MDS 2.0 resource for all team members in the care community.
• Establish an effective process for auditing RAI MDS accuracy and compliance with coding, rapping and care plan updating.
• Review and analyze monthly RAI MDS for each resident to ensure accuracy of assessments and meeting resident needs.
• Manage the computerized Data Submission process and maintain records within Ministry timeframes.
• Monitor and analyze RAI outputs and reports and communicate with management and committees.
• Perform other duties as assigned.
